You don’t need to keep sliding, you just need to be sliding for the time when you actually hit the heavy attack button. Also to not switch to your main weapon, you need to actually switch to your melee, not the quick swap.

I find it preferable to using a glaive as they're pretty clunky on console. Your mileage may vary. I use a sepfahn with exodia contagion and can officially say it’s by far the most fun melee option for me.
